Expression of Interest for the role of Co-Chair

A Regional Social Prescribing Development Board is being established by the Health & Social Care Board and the Public Health Agency, which will be co-chaired by the Director of Integrated Care at the HSCB and a senior leader from the Community & Voluntary Sector.

Expressions of Interest for the Co-chair role are welcomed from those who can demonstrate that they meet the criteria as detailed in the documents below:

–         Expression of interest template for completion

–         Declaration and Register of Interests

Expressions of interest and the declaration and register of interest should be submitted using the templates provided by email to lorna.marshall@hscni.net by 5pm on Friday the 9 of October.
